• Personalisation: 
  OPTION 1. Template Personalisation (FREE). Select this option and we will personalise your selected Dental Lab Dockets template to your needs. Order now and you can attach your company information and any logo/images you would like to include. Once we receive your order, we will customise the template and email you a digital proof to approve before printing. 
  OPTION 2. Attach Your Pre-Designed File (FREE). Select this option if you already have the personalised design you would like us to use for your order. Attach your PDF design file when you place your order. We will email you a digital proof to approve before printing. 
  OPTION 3. Our Design Service (+£28.00). Select this option if you require our design services. You can attach your company information and any logo/images you would like to include as you order. We will email you a digital proof to approve before printing your order. (Please note, if your design requirements are particularly complex there may be an additional charge for design work and we will contact you before starting any work. Our standard charge allows for up to 1 hour design time in our studio). 
• Full Colour Printing. Printed in as many ink colours as you require. This is perfect if you have a full colour logo for instance. 
• Prices are for printing to the front of all sheets. If you also require printing on the back of the sheets (eg; terms and conditions), please ask for a FREE Quote 
• Available as: 
  Duplicate (Standard: 2 Part NCR White/Yellow Carbonless Paper) 
  Triplicate (Standard: 3 Part NCR White/Yellow/Pink Carbonless Paper) 
  Quadruplicate (Standard: 4 Part NCR White/Blue/Yellow/Pink Carbonless Paper) 
  (If you require a different NCR paper colour sequence to our standard, we can do this at no extra charge. Please let us know your required paper colour sequence in 'Special Instructions' as you place your order. We stock the following NCR paper colours: White, Blue, Yellow, Pink, Green. Please note: the top NCR sheet must be White). 
  
• Sizes Available: 
  A5 (148 x 210mm) 
  A4 (210 x 297mm) 
  Custom sizes are also available on request 
  
• Prices are for Glued NCR Pads with 50 NCR Sets per Pad. As standard, we glue the pads at the top. If you would rather have your NCR Pads glued on the left hand side, please let us know in 'Special Instructions' as you order. We can also supply your order as NCR Books (Perforated, Stapled and Bound with a Stub) or NCR Sets (Individual), if required. Please contact us if you would rather have your forms supplied as NCR Books or NCR Sets. 
• Sequential Numbering can be added to your NCR printing order. We charge a standard fee of £15.00 for Sequential Numbering. Numbering will start from a number of your choice. Numbering is sequential, for example 0001, 0002, 0003 etc. You can let us know your chosen start number as you place your order. 
  
• Each Pad has a grey board backing card. 
• Loose Writing Shield Cards supplied free of charge with your order. 
  
• Hole Punching is free of charge if required. Let us know if you require Hole Punching in 'Special Instructions' as you place your order. 
  
• Custom printed NCR Pads are a professional and efficient way of keeping your business affairs in order. When you write on the top NCR sheet of paper, an instant copy of what you have written is automatically made onto subsequent NCR sheet(s) below.
